All trips require the following Group & Flight Information as well as the Blue Sky Adventures General Release Forms.

Each person on the trip needs to bring the Blue Sky Adventures Release Form (regardless of the itinerary —DO NOT MAIL THEM TO US!)
Select the release forms that pertain to your trip, print one copy per participant, and bring the completed forms with you–DO NOT MAIL THEM TO US!
